Position Summary: Delivering adult transition, work-based learning experiences, engaging student interactions with industry or community professionals in real, virtual, online, or simulated work environments that expose learners to postsecondary options. Provide opportunities for work habit skill development and proficiency attainment, allowing students to reinforce and deepen their school-based learning. These opportunities would particularly help students make the connection between academic principles and real-world applications. For many, understanding “Why do I need to know this?” provides motivation for more learning.
Job Functions and Responsibilities:
General Responsibilities:
- Support Special Education Student’s Transitional plans through instruction and programming opportunities.
- Provide mentoring to students, including role modeling appropriate work habits and inspiring students to plan and work towards their future education and work goals.
- Provide instruction for students to develop and improve their social skills, interpersonal skills, and coping strategies with opportunities to practice these skills.
- Communicate and collaborate with all treatment team members in a professional and productive manner.
- Complete all necessary paperwork and grading in a professional and timely manner.
- Other duties as assigned.
Specific Responsibilities:
- CLC will work with 3-4 students daily and is responsible for the development, coordination, implementation, and evaluation of a range of work-based learning experiences that take place both inside and outside of the school
- Inspire students to engage and participate in adult living and consumer science programming activities and initiatives
- Enter professionally written case notes each day identifying student services provided
- Establish collaborative relationships with community organizations, businesses, and event organizers 2 of 3
- Collaborate with the student’s treatment team to proactively identify student needs related to transitioning to a step-down or community residence including school transitions, programming opportunities, job opportunities
- Coordinate with Permanency Coordinators to have student permission slips for events signed and returned in a timely manner
- Identify student’s relational resource needs to assist students in identifying people they need to support them to accomplish their transition goals and objectives
- Provide written reports of student’s transition plan progress to the Academic Case Manager and Permanency Coordinator to represent student plans and progress in IEP and ITP meetings
- Be familiar with Choices 360 and Casey Life Skills assessments and knowledge of how to locate completed student assessments
- Identify students with IEP Transitional Plans – Students 14 and over
- Instruct and support students ready for internship or school-to-work
- Facilitate and identify potential work sites for SWP students
- March, April, May, and June will facilitate one community service event per month for students and faculty
- CLC will support students and employers in planning and implementing safe, effective, and authentic learning experiences as identified in the Extended Learning Opportunities (ELOs) and the seven standards for work-based learning
- School-to-work opportunities are continuously updated based on stakeholder evaluation data, current and emerging professions, and economic trends as identified by the Regional and Local Advisory Boards
- Support and help facilitate student-run businesses and entrepreneurship opportunities on campus
- CLC will be an invaluable team member collaborating with ACMs, Classroom Teachers, Permanency Coordinators, Clinicians, and external team members in supporting existing and long-term goals for students in relation to Adult Living and Transitions from school
Required Education:
- Bachelor’s degree
- Preferred: NH DOE Career and Technical Education Teacher (CTE) Certification
(Must be willing to pursue NH DOE licensure)
Required Qualifications and Experience:

- Minimum of three years of experience in education, business/nonprofit operations, and career services.
- Experience as a teacher, counselor, or school administrator in middle or high school (preferred)
- Experience related to local and/or state graduation proficiencies, proficiency-based assessment, proficiency-based portfolio development, using data to inform decision-making, and other best practices in education.
- Working knowledge of career readiness and preparation, including the development of flexible personal learning plans as indicated by the NHDOE.
- Strong collaboration and communication skills
- Self-starter, flexible, and willing to work beyond the school day schedule. 3 of 3
- Innovative thinker
- Knowledge of the economic and social landscape of Keene, NH.
