Summary And Essential Functions
Schedule: 40 hours/week. Day shift schedule. No nights, LRH observed holidays or weekends.
Essential Functions
Responsible for assessing the nutritional needs of the inpatient and outpatient populations and encouraging appropriate nutritional diagnoses with physician collaboration.
Responsible for developing, implementing, and monitoring nutrition care plans to promote optimal nutrition for the best health and wellness outcome possible. Communicates patient needs and interventions with appropriate staff.
Documents nutrition care plans, recommendations, and interactions with the patient and any other health care personnel or agency as per professional standards and LRH requirements. Maintains these records in a format that adheres to recommended state and Medicare guidelines.
Provision of follow-up care as indicated to provide patients with skills and information they can utilize to the best of their ability to maintain and/or improve nutrition and wellness, as well as assist with obtaining support to promote healthy nutrition (such as Home Health aides, diabetes clinic, WIC program, and other local community support agencies).
Deliver medical nutrition therapy (MNT) to the outpatient population via multiple modalities to help mitigate patients negatively impacted by social determinants of health. Patient appointment modalities include in person, telehealth and group settings.
Responsible for understanding criteria required for referrals and the necessary billing codes for services rendered.
Utilizes technologies including the electronic health record (EHR), telehealth technologies and other virtual platforms as needed.
Maintains registration eligibility as per guidelines set by the Commission of Dietetic Registration and licensure by the State of New Hampshire.
Reviews Centers for Medicare and Medicare quality measures within the food and nutrition department including updating manuals such as policy and procedures, business plan, policies, diet manual and continuing education manual. Conducts in-services as needed to provide nutrition knowledge to department employees in areas needed to meet inpatient diet needs.
* Participates in hospital sponsored Health and Wellness fairs, community programs, and other community activities as needed.
Note: The items marked with * are not required to be performed by the per diem dietitian unless requested by Director of F & N or full-time clinical dietitian.

SUPERVISES: N/A
INTERNAL AND EXTERNAL CONTACTS: Patients, families, physicians, and all other LRH employees and referring providers.
Qualifications
Required Education/Course(s)/Training:
Master of Science Degree in Nutrition/Dietetics from an accredited college/university
Registered by the Commission on Dietetic Registration
Licensed by State of New Hampshire
Preferred Certification/Registration
ADCES Certified Lifestyle Coach (training to be completed within 12 months of hire)
CDCES encouraged
